Diabetes Running Health Record is a collection of documents that can be used to provide a patient with a folder in which they can keep information (test results, educational material, targets) relating to their condition. The 3 main documents required to produce a running health record are available below.
1. Diabetes Master Oct 2008 – This doc forms the basis of the folder and contains information about the practice, the patient, how the use the folder, GP Management Plans.
2. Annual Cycle of Care Checklist – Is a Form that is used to ensure all elements of the Annual Cycle of Care are scheduled and completed.
3. Targets for Good Health – is a table that shows for each of the key measures, what the targets are so that the patient knows how they measure up.
