Australian Primary Care Collaboratives (APCC)
Overview
This program commenced in 2008 and is a quality improvement program for General Practice that utilises data and a collaborative methodology with the primary aim of improving health outcomes for patients with chronic disease. There are 2 local practices taking part in this federally funded initiative (Vale Medical Clinic and Pittwater Family Practice).
Aims
The 3 focus areas include; 1. Improving access to General Practitioners by analysing practice capacity, 2. Increasing the % of patients with diabetes type 1 or 2 with an HbA1c of 7.0 or less (target is 50%), 3. A 30% reduction in the mortality of patients with coronary heart disease in 3 years. The Division expects to take the learnings from the APCC program and apply them at a local level in the next 12 months.
Activities
• Regular improvement activities called PDSA’s that focus on making small steps towards an improvement to be implemented in the practice.
• Monthly data collection to measure how these improvements are impacting the practice.
• Learning Workshops to share ideas and suggestions for improvement.
• Local meetings to plan future activity and share learnings and experiences.
